Advanced Academic Program (AAP) forms, purpose, and deadlines

For your convenience, all AAP forms are available below along with the purpose of the form, who completes the form, and the deadline. All forms are also linked within pages about AAP services.

AAP School-Based Services (Level II-III) Referral Form

Purpose: To apply for school-based Level II or Level III AAP services (Grades K-6)

Who: FCPS Parents/Guardians submit to the school

Deadline: Submit at any time during the school year to the Advanced Academic Resource Teacher (AART)

AAP Full-Time (level IV) Referral Form

Purpose: To request screening for AAP Full-Time (level IV) services (Grades 2-7)

Who:

  • Current FCPS Students: Parents/Guardians submit to the Advanced Academic Resource Teacher (AART) at the local school between first day of school and December 15 to request spring screening
  • Newly Enrolled FCPS Students: Parents/Guardians submit by the Fall Screening Deadline, October 15 to request fall screening
  • Private/Home School Students: Parent/s Guardians submit as part of screening file

HOPE Rating Scale

Purpose: To provide screening committees with a rating scale that measures two very broad components of giftedness and talent: Social and Academic.

Local school committees submit one HOPE Rating Scale per student.

Who:

  • Current FCPS Students: The local school completes for each student referred for Full-Time Advanced Academic Services.

    Form posted for FCPS parent information only.
  • Private/Home School Students: Current teacher(s) completes the form; Parents/Guardians submit the form with the application file.

Deadline: See published deadline for private/home school screening applications

Parent/Guardian Questionnaire

Purpose: Optional form to provide information about the student from the family to support the AAP identification process for Levels II-IV

Who:

  • FCPS Parents/Guardians submit to school along with appropriate referral form
  • Private/Home School families submit with screening file

Deadline: Meet all relevant referral/submission deadlines

AAP Full-Time (level IV) Summary Sheet

Purpose: Cover sheet for screening files

Who:

  • Current FCPS Students: The local school completes for each student referred for Full Time (level VI) services. Form posted for FCPS parent information only.
  • Private/Home School families submit with screening file

Deadline: See published deadline for private/home school screening applications

Advanced Academic Programs Full-Time (level IV) Transfer/Reactivation Form

Purpose: To request:

  • reactivation of Full Time (level IV) services, or
  • a change in school placement for the upcoming academic school year

Who: Parents/Guardians of Full Time (level IV)-eligible students complete for students who:

  • previously deferred services and would like to reactivate,
  • moved from/returned to FCPS, or
  • would like to change placement from the Local Level IV school to the Level IV Center school or vice versa

Deadline: Submit to AAP office by May 15. Please scan or take a clear photo of your form and submit the form to [email protected]. Submission of this form represents a one-year commitment After May 15, the school placement may not be changed.

AAP Appeal Form

Purpose: To appeal Level IV eligibility decision for the most recent screening cycle

Who: Parents/Guardians mail form and additional materials to AAP office according to directions on the form

Deadline: Must submit by appeals deadline
