Student Health Information Needed for Return to School

Update information online

The Health Information Form (HIF) and the Emergency Care Information form provide valuable information regarding student health conditions. These forms are critical in identifying students who may need healthcare support while at school or on the bus. Parents/guardians are reminded that they can provide this information online via their FCPS 24/7 (Blackboard) accounts within the weCare@school (weCare) module.

Public Health Nurses (PHN) utilize the information contained in the HIF to develop Healthcare Plans (HCP) and Action Plans and to develop teaching strategies for FCPS staff to support students. Parents/guardians of students with existing HCPs or Action Plans will need to get updated physician’s orders, as necessary, prior to their child’s return to school. If you have questions about the need for updated physician’s orders, please contact the Public Health Nurse at your child’s school.
