FCPS permission forms for athletes who use an
Inhaler
or Epi-Pen
Physician release allowing athletic participation
while wearing a cast.
Note: At this time, VHSL rules limit
participation with a cast to football and soccer players
only, where the cast is below the elbow. If you feel it
is appropriate for an athlete to continue participation
in sports during the later stages of fracture healing,
the appropriate cast release form must be completed by
the physician and kept on record with the school's ATC.
