Forms Needed for Participation
Several forms are required for participation in FCPS high school athletic programs.
These forms and certificates are applicable at every school:
- Physical Form
- Weight Room Permission Form
- Student Emergency Care Form
- Communicable Disease Form
- Concussion Education Program
- Extracurricular Participation Pledge - check with your local school to confirm if additional information is necessary
Other forms may be required according to individual school or student needs. Examples would be:
Driver’s License and Vehicle Information
Student Accident Insurance
(FCPS requires insurance in order to participate in athletics. If you have personal health insurance, that is sufficient. If you do not, it can be purchased).
