Review Regulation 4621, Salary Guides–Credit Beyond Bachelor’s Degree. Please note the guidelines for eligibility and acceptable course credits.
Review and complete the Additional Salary Request Form.
Submit the Additional Salary Request Form along with appropriate support documentation to the Office of Licensure.
