In an effort to conserve paper and manpower, Westfield High School will open “virtually” this year. Below you will find all the forms that you MUST fill out, sign, and return to school. In addition, you will find optional paperwork that can be returned depending on your choices for your student.
The following Three essential forms MUST be downloaded, filled out and returned to your student’s seventh period teacher on the first day of school. Please type directly onto the form first, then print form, sign the form, and finally return the completed form to your student’s seventh period teacher on the first day of school. Student’s locker will not be assigned until the completed forms are returned to school.
