NEW STUDENT REGISTRATION
PARENTS/GUARDIANS MUST SCHEDULE A "NEW STUDENT REGISTRATION" TO ENROLL STUDENTS WHO ARE CURRENTLY NOT ATTENDING ONE OF STUART'S PYRAMID SCHOOLS (GLASGOW, POE, OR HOLMES)
New Students who are currently not attending one of Stuart’s pyramid schools (Glasgow, Poe, or Holmes) and who are planning to attend Stuart need to call Mrs. Cathy Ramos at 703 824-3922 to schedule an appointment between the hours of 7am-3:30pm, Monday-Friday.
NEW STUDENTS WILL NOT BE ADMITTED WITHOUT A PRIOR APPOINTMENT!!
To enroll a student, a Parent/Guardian must provide all of the following required items:
- The natural or adoptive Parent or Guardian and the New Student must be present
- A Photo ID of the Parent/Guardian
- A valid/official Birth Certificate of the New Student
- Proof of Residency in Fairfax County - a lease/deed/rental agreement.
- If living in the residence of a friend or relative, the homeowner/leaseholder must provide a notarized affidavit accompanied by the lease/deed/rental agreement
- Social Security Number
- Original transcripts, withdrawal grades, report cards, and/or transfer slips from the previous school(s) the student was attending
- Certificate of a recent (within 12 months) Physical Examination by a Licensed Physician for students in grades K-6 and all foreign students (K-12).
- United States transfer students should provide a record of their Physical from their previous school.
- Evidence of a Negative Tuberculin Test or a Normal Chest X-Ray administered within 90 days of all students who have resided in a foreign country (with the exception of Canada, Western Europe, Australia, and New Zealand) during the last Three (3) years for a consecutive period of three or more months
- Evidence of other required immunizations
- Virginia Law requires that every new student be immunized against: Diphtheria (DPT), Pertussis, Tetanus, Polio, and Measles/Mumps/Rubella (MMR)OR
- Provide official statements from a Licensed Physician explaining why the Diphtheria (DPT) and/or Measles/Mumps/Rubella (MMR) immunization was not given
- Students born on or after January 1, 1994 are required to have been immunized with the Hepatitis B vaccine (HBV).
- Students lacking any of the required documentation of immunizations will not be able to register and/or attend.
- Objections based on religious grounds may be discussed with the Principal.
- New Student Registration Forms - These forms are found either in a "bundled" format or as individual forms. The forms work best if you have Adobe Acrobat Version 6. You may complete the forms online or print them out and fill them out by hand.
