Registering New Students: The Basics
Parents are asked to contact the guidance secretary, Ms. Cathy Ramos, to arrange for an appointment. Students will normally begin school the day after registration. To enroll a student, a parent or legal guardian must provide the following required items:

• The presence of a natural or adoptive parent and the student
(A photo ID of the parent may be requested.)
• Birth certificate
• Proof of residency in Fairfax County, generally a lease or a deed. If living in the residence of a friend or relative, the homeowner / leaseholder must provide a notarized affidavit accompanied by the lease or deed.
• Social Security Number (requested)
• Transfer slip or report card from the previous school and/or original transcripts
• Certificate of a recent (within 12 months) physical examination by a licensed physician for students in grades K-6 and for all (K-12) foreign students
• Evidence of a negative tuberculin test or a normal chest X-ray administered within 90 days for all students who have resided in a foreign country (with the exception of Canada, Western Europe, Australia, and New Zealand) during the last three years for a consecutive period of three or more months
• Evidence of other required immunization (Virginia law requires every new student to be immunized against diphtheria, pertussis, tetanus, polio, measles, mumps, and rubella or to provide statements from a doctor explaining why the DPT and/or MMR immunization was not given.) Students born on or after January 1, 1994, are required to have been immunized with the hepatitis B vaccine (HBV). Students lacking required documentation of immunizations will be barred from attendance. Objections based on religious grounds may be discussed with the principal.
• Physical examination for all foreign born K-12 students must be provided. US transfer students should provide a record of a physical from a previous school.

Registering New Students: The Forms
For the convenience of parents and guardians who fill out FCPS forms, there are two different form “bundles” on the main FCPS web site that group multiple parent forms together. These groupings allow the data entered on one form to be re-used on all other forms in the bundle, reducing the number of times that parents have to enter the same information if they have more than one child in FCPS. There are separate bundles of forms for new enrolling or re-enrolling students and for continuing students (those returning after summer break).

The URL to access the form bundle is http://www.fcps.edu/form.htm. From there, parents or guardians can select two form bundles or select individual forms for their use. After completing the forms online, parents can print them for use when needed.
