Registration Information
To register a student in Fairfax County Public Schools, parents must provide the following information:
1. Proof of Residency:
A deed, lease agreement, or resident manager's letter is acceptable. If a family is living in the residence of a friend or relative, the homeowner/leaseholder must provide a notarized affidavit accompanied by the lease or deed.
The student must be accompanied by the person with whom he or she lives, either a parent or court-appointed guardian or legal custodian. The accompanying adult may be required to present a photo identification card.
Physical presence of the parent in the residence of the child is required for tuition-free enrollment in FCPS. A change in enrollment status may be necessary if a parent initially enrolls the student but leaves Fairfax County.
2. Proof of birth/citizenship:
An original birth certificate is required for all students. If a birth certificate is not available, an affidavit with substantiating documentation i required.
The legal name of the student as shown on the original birth certificate will be entered in the student record. Changes to an original birth certificate must be supported by a court document.
3. Documentation of a previous educational program:
School records and/or original transcripts are required of secondary students and recommended for elementary students. Recent periods of non-school attendance must be accounted for. If a translated document is submitted, the original document in the original language must also be supplied.
Schools will send all non-U.S. school transcripts to Student Registration for evaluation and translation when necessary.
4. Health Documentation:
Physical Exam: A certificate of physical examination, completed within the last 12 months prior to starting school, by a licensed physician, licensed nurse-practitioner or licensed physician assistant acting under the supervision of a license physician is required for all students being admitted for the first time in kindergarten or elementary school. Transfer students must provide either a record or a physical examination dated within the last 12 months.
Immunization Requirements: Official documentation including complete dates (month, daty, year) is required for DPT, DTaP, Td, DT (diptheria, pertussis, tetanus), OPV, IPV (polio), MMR (measles, mumps, rubella) Hepatitis B, Varicella (chicken pox) and HIB (Haemophilus influenza type B).
Tuberculosis Screening: For all students who have resided in a foreign country (with the exception of Canada, the countries of Western Europe, Australia and New Zealand) during the last 5 years for a consecutive period of 5 months or more, one of the following is required--evidence of a negative tuberculin test, evidence of a normal (negative) chest X-ray taken within 90 prior to registration, documentation of a negative symptom screen and risk assessment from a health department or private physicial for students who present written documentation of having completed prophylaxis treatment
