GREETINGS FROM OAK HILL SCHOOL HEALTH TEAM

Our goal is to ensure that your student will enjoy a safe and healthful school year. Please read the following information carefully, and feel free to call your School Health Aide, Vicki Franks, at 703-467-3510 or your School Public Health Nurse, Holly Brenneman, at 571-722-5778 if you have any questions.

YOUR SCHOOL HEALTH SERVICES INCLUDE:

• Care of sick and injured students and administration of medication according to Fairfax County Public School guidelines
• Coordination of care for students with identified health concerns
• Identification and follow-up of communicable disease
• Vision and hearing screening for K, 3rd, and new-to- county students

HOW CAN YOU HELP US PROVIDE THE BEST POSSIBLE CARE?

• Complete and return the Health Information Form, listing specific health conditions that may impact your child’s school day. A medical alert list is developed using this information to inform each child’s teacher(s) of health issues.
• Complete and return the Emergency Care Information Form, listing as emergency contacts someone who can be reached and can pick up your child should he/she become sick or injured at school. Notify the school office of any changes to the information provided on this form.
• If your child will have PE restrictions, notify the PE teacher directly.
• Keep a sick child home. Check your child before he/she leaves for school in the morning. It is difficult to concentrate and perform tasks when not feeling well.

Medication Guidelines:

Medication should be taken at home whenever possible. This prevents the student from losing valuable classroom time or having a shortened lunch period. If medication is to be administered at school, specific guidelines must be followed in accordance with the FCPS medication policy. Students may not take medication at school unless guidelines are followed.

1. Parent or guardian may authorize administration of the following medications at school by completing, signing and dating Parts l & ll of MEDICATION AUTHORIZATION FORM:

• Over-the-counter medication for pain relief may be given on an as needed basis throughout the school year for headaches, muscle aches, orthodontic pain, or menstrual cramps.
• Over-the-counter medication for conditions other than those above may be given up to ten consecutive days.
• Antibiotics and anti-viral medication

2. Physician must complete, sign, and date the order on Part ll of the medication forms for all other medications other than those situations noted above. Part l of the authorization forms must be completed and signed by the parent or guardian.

3. Cough drops and throat lozenges may be carried and used by students as needed throughout the school year, provided they are in the original container or package. Students must not share cough drops or throat lozenges under any circumstances. No medication authorization form is required.

4. Written orders in Part ll of the medication forms must include:
• Diagnosis
• Name of medication-no abbreviations
• Dosage
• Time at which the medication is to be taken
• Sequence of medication when more than one is prescribed
• The condition or symptom that requires repeating medication
• Interval between doses-repeat as necessary is not acceptable
• Duration of medication order

5. Medication containers must be properly labeled.
• All prescribed medication, including physician samples, must be in a container labeled and dated by a pharmacist or physician with the name of the student, name of medication, dosage, and time to be taken.
• All over-the-counter medication must be in the original container and labeled by the parent or guardian with the name of student, dosage, and time be taken.

6. The first dose of any new medication should be given at home.

7. The parent or guardian must transport medication to and from school.

8. Each time there is a change in the dosage or time, a new authorization form is required. The new medication order and container label must match.

9. Students must go to the health room to receive medication. Parents should develop a plan with the student to ensure that he or she goes to the health room at the prescribed time. Medication can only be given one half hour before or one half hour after the prescribed time.

10. Parent or guardian must pick up any unused portion of the medication within one week after expiration of the physician order or on the last day of school. Medication not claimed within that period will be discarded.

11. At the beginning of each school year a new medication form is required.
