YOUR SCHOOL HEALTH SERVICES INCLUDE: • Care of sick and injured students and administration
of medication according to Fairfax County Public School guidelines HOW CAN YOU HELP US PROVIDE THE BEST POSSIBLE CARE? • Complete and return the Health Information Form, listing
specific health conditions that may impact your child’s
school day. A medical alert list is developed using this information
to inform each child’s teacher(s) of health issues. Medication Guidelines: Medication should be taken at home whenever possible. This prevents the student from losing valuable classroom time or having a shortened lunch period. If medication is to be administered at school, specific guidelines must be followed in accordance with the FCPS medication policy. Students may not take medication at school unless guidelines are followed. 1. Parent or guardian may authorize administration of the following medications at school by completing, signing and dating Parts l & ll of MEDICATION AUTHORIZATION FORM: • Over-the-counter medication for pain relief may be given
on an as needed basis throughout the school year for headaches,
muscle aches, orthodontic pain, or menstrual cramps. 2. Physician must complete, sign, and date the order on Part ll of the medication forms for all other medications other than those situations noted above. Part l of the authorization forms must be completed and signed by the parent or guardian. 3. Cough drops and throat lozenges may be carried and used by students as needed throughout the school year, provided they are in the original container or package. Students must not share cough drops or throat lozenges under any circumstances. No medication authorization form is required. 4. Written orders in Part ll of the medication forms must include: 5. Medication containers must be properly labeled. 6. The first dose of any new medication should be given at home. 7. The parent or guardian must transport medication to and from school. 8. Each time there is a change in the dosage or time, a new authorization form is required. The new medication order and container label must match. 9. Students must go to the health room to receive medication. Parents should develop a plan with the student to ensure that he or she goes to the health room at the prescribed time. Medication can only be given one half hour before or one half hour after the prescribed time. 10. Parent or guardian must pick up any unused portion of the medication within one week after expiration of the physician order or on the last day of school. Medication not claimed within that period will be discarded. 11. At the beginning of each school year a new medication form is required.
