Athletics/Activities

Extracurricular Participation Policy
All students who participate in extracurricular activities must sign the "Participant's Pledge" or the Acknowledgement as a precondition of his/her participation. The student's signature on the Pledge/Acknowledgement signifies the commitment to abide by the conditions of the Marshall High School Extracurricular Participation Policy and to remain free of alcohol, tobacco, and illegal substances. The parents signature on the pledge signifies that the parent has read and understands the pledge.(3 pages)

VHSL Physical Exam Form
The athletic participation/parental consent/physical examination form required for athletic eligibility. (4 pages)

Guest approval form
Required to bring guests to GCM-sponsored activities, such as the homecoming and prom dances.

Commonwealth of Virginia School Entrance Health Form
This form is required for admittance to Virginia schools. (4 pages)

Emergency Care Form
All students must submit a completed emergency care sheet each year. Information on this sheet will be used to contact parents/guardians in the event of an emergency. (1 page)

Epi-Pen Authorization
Epinephrine may be given in school, during school-sponsored activities, or at SACC only with a parent- or guardian-signed authorization, including a signed physician's statement. (2 pages)

Inhaler Authorization
Any medication taken in school must have the parent- or guardian-signed authorization and physician order if required by regulation. A new form must be submitted at the beginning of the school year. (2 pages)

Medication Authorization
Any medication taken in school must have a parent- or guardian-signed authorization; some medications also require physician orders. Medication must be kept in the school clinic or other school approved location during the school day. (2 pages)

Student Health
This health information sheet must be completed by parent or guardian each school year. (2 pages)

Acceptable Use Policy (AUP)
To use GCM's information systems, all students must have a current AUP form on file. Information systems include, but are not limited to, hardware, software, communication lines and devices, graphing calculators, terminals, printers, CD-ROM devices, scanners, digital cameras, LCD projectors, and computers.
