STEPS:

1. PARENTS complete the required permission form below

2. STUDENTS bring in permission form and device on a DESIGNATED date which will be announced in advance

Important!To bring in devices to be checked and approved: Bring your device and paperwork to 101E BEFORE SCHOOL, During LUNCH, or During LancerTime every FRIDAY!
