The Clinic provides many services to Lee High School students. In addition to taking care of sick and injured students, the Clinic Aide works with the school in planning annual hearing and vision screenings. Parents can help the Clinic provide good service by noting and adhering to the following procedures:
| Checking out ill students |
Students who don't feel well during the school day should ask their classroom teacher for a pass to the Clinic. If the student needs to leave school because of the illness, the Clinic Aide will call the parent/ guardian listed on the Emergency Care Card. Students should not call parents from classroom phones or personal cell phones during the school day as this can result in disciplinary action. Students will not be checked out of school without authorization from the person(s) listed on the Emrergency Care Card. Please keep this information up to date and let the school know of any changes in phone number, authorizations, address, etc. Students who leave school without prior authorization will receive an unexcused absence for the time missed. For the health of the Lee community, please do not send your sick student to school as illnesses can spread quickly in a school environment.
| Health Information Form |
Please complete and return the Health Information Form at the beginning of each school year. This is the way for parents/ guardians to alert the Clinic of any medical or health conditions that may affect your student during the school day.
| Prescription Medications |
Parents and the physician need to complete and sign the Medication Authorization Form and the medication needs to be brought to the school by a parent. Medication must be in the labeled pharmacy container. Parents/ guardians of students who need an inhaler at school should complete and return the Inhaler Authorization Form. Parents/ guardians of students who need to have Epipen at school should complete and return the Epipen Authorization Form.
| Over-the-Counter Medications |
The Clinic does not stock or dispense any medication without parental consent given on the Medication Authorization Form. The parent/guardian or student may bring these medications to the clinic in their original container.
