EMERGENCY CARE CARD
The purpose of the Emergency Care Card is to locate the
parent/guardian of a child in the event of illness or emergency.
WE MUST BE ABLE TO LOCATE YOU AT ALL TIMES DURING THE SCHOOL
DAY. Any change in the information that you provided on the card
since the beginning of the year should be sent to the office as soon
as possible (moving, new job, new phone number, new babysitter,
etc.).
Please list the phone numbers of two emergency contacts who will
know where you are for emergency calls and who may pick up your child
when he/she is ill and you are unable to pickup your child.
DAY CARE FACILITIES
A list of local daycare facilities is available in the office for
your convenience.
