Back to School Forms

In an effort to conserve paper and manpower, Herndon High School will open “virtually” this year.  Below you will find all the forms that you MUST fill out, sign, and return to school.  In addition, you will find optional paperwork that can be returned depending on your choices for your student.

Please remember each form must be filled out for each individual child who is attending Herndon High School for the 2011-12 school year.

For any 10th, 11th, and 12th grade student to receive his/her schedule, the following forms must be returned on schedule pick-up day:
1. Emergency Care Form
2. Health Information Form
3. Signed parent/student understanding of Student Responsibilities Form

Schedule pick up times and dates for 10th, 11th, and 12th graders only:
August 31           Schedule pick-up – cafeteria – 8:00 am – 3:00pm and 6:00 pm – 8:00 pm
September 1      Schedule pick-up – cafeteria – 8:00 am – 12:00 pm

Any student who does not have his/her schedule by the first day of school may pick up his/her schedule from the Guidance Office.  Students who have not returned their forms by the first day of school must return the required forms to their 3rd period teacher by September 9, 2011.

Freshman- will receive their schedules at Freshman Orientation (pdf) on August 30, 2011.  Their forms may be handed to their counselor that day or returned to their 3rd period teacher the following week.  Again all forms are due by September 9, 2011.

Please note that parents who wish to opt-out students should complete the appropriate forms and return them to their child’s 3rd period teacher or on schedule pick –up day with Health and Emergency Care forms.

• Objection to Release of Directory Information to the Public (p. 7)
• Objection to Release of Information for Commercial Purposes (p. 8)
• Denial of Access to Military Recruiters (p. 8)
• School Counseling Opt-Out Form (p. 9)
