Federal Student-Parent Survey Forms
Federal Student-Parent Survey forms were given to parents at parent teacher conferences or were sent home in Thursday folders. Please complete, sign, and date the survey and return it to your child’s school as soon as possible.

We must have a form returned for every student.

Impact Aid is a federal grant program that was designed to lessen the impact of federal activity on the operating budgets of local school systems.  FCPS receives approximately $3,500,000 annually from this grant program.  These funds become a part of the school system’s general operating budget and are critical to sustaining the high quality of education that FCPS provides for your children.

The information that you provide will be treated confidentially.

Each student will receive a colorful gel pen upon receipt of the signed Federal Survey form.
