You can submit this form electronically by clicking the "Submit by email" button in the upper right corner, or you can print the completed form and send it to the Main Office. If you are having trouble accessing the fillable form, you can download a regular pdf form here. Forms are due by June 1st.

Haycock Elementary School has again been awarded the Golden Apple Wellness Award for 2011 from the Fairfax County Public Schools Wellness Task Force. The Golden Wellness Award is the top tier in the GOLDEN Wellness Series (Giving Others Lifelong Desire for Exercise and Nutrition). Haycock was successful in promoting student health and contributing to the reduction of childhood obesity.
