Online Student Forms

If you have children in Fairfax County Public Schools, it's almost a certainty that you have suffered writer's cramp from filling out the many forms that come home at the beginning of each school year.

For your convenience, the Department of Instructional Technology has created sets of forms, referred to online as "bundles", which you may download, complete and print on your home computer. The bundles allow data entered on one form to be reused on all other forms in the group, reducing the number of times you have to enter the same information. Forms for children who need to use inhalers or take medication at school are also included for use as needed. If you have more than one child in Fairfax County Public Schools, there is even a feature to reset the forms and change only the child's name.

  • Go to http://www.fcps.edu/forms.htm.
  • Choose the category that fits you as either a newly enrolled in FCPS or a returning student.
  • Follow the prompts to complete the forms that you need.

Franklin Middle School will still send home paper copies of the forms on the first day of school, recognizing that not all families have access to a computer. If you have downloaded and printed the forms at home, discard the paper copies.

All Franklin Middle School students must complete an Emergency Care Card, Pupils Riding School Buses form, Health Information Form and a Network Access Policy form. 

Please be sure and include your child's bus number if they ride a bus. Your child's student ID number can be found on their schedule or an old report card.

Print and sign two (2) copies of the Emergency Care Card. Only one copy of the other forms are needed.

Suggestion: When you print the forms, use your printer's "Print Page" feature rather than printing the entire document. (The form "bundles" include 16 pages and many families will not need the medications/inhaler forms.)
