Welcome to Franklin Middle School, a Fairfax County Public School. Our mission, in collaboration with the school community, is to provide students with challenging instructional programs in a safe and nurturing climate. At Franklin, students develop communication skills, confidence, competence, creativity, and compassion while gaining enthusiasm for learning as a lifelong endeavor.

Individual Forms:

The student must be accompanied by the person with whom he/she lives, either a parent or a court-appointed guardian or legal custodian.  Accompanying adult may be required to present a photo identification card.

The following registration procedures have been developed to ensure a smooth transition and optimal placement and scheduling for your child.  To register a student parents must provide:

  • Proof of Residency: A deed, lease agreement, sales contract or resident manager's letter is acceptable
  • Proof of birth/citizenship: An original birth certificate is required for all students
  • Social Security Number: Evidence of federal social security number is requested for United States citizens and non-U.S. citizens who hold documentation of permanent residency.
  • Documentation of previous educational program: School records and/or original transcripts are required
  • Health documentation: Immunization Requirements: Official documentation including complete dates (month, day, year) is required for the following:

    *Diphtheria/Pertussis/Tetanus (DPT, DT, DtaP, Td): a minimum of three doses are required at least four weeks apart, with one dose administered after the fourth birthday. unless the child has received six doses of DPT,

    *Poliomyelitis (OPV, IPV):  a minimum of three doses are required at least four weeks apart, with one dose administered after the fourth birthday unless the student has received four doses of OPV or IPV before the fourth birthday.

    *Measles, Mumps, Rubella (MMR): A minimum of two measles, one mumps and one rubella (most children receive two doses of each because the combination vaccine is used) is required. 

    *Hepatitis B: A minimum of three doses for all children born on and after January 1, 1994 with at least four months spacing between the first and third dose. 

    *Varicella (chicken pox): For all children born on or after January 1, 1997, one dose of varicella is required, administered not earlier than the age of 12 months. Dated report of serological confirmation or statement from physician of a reliable report of the disease is acceptable.

    *HIB (Haemophilus influenzae type B): age appropriate schedule for children up to 30 months of age.

*Tuberculosis screening requirements: For all students who have resided in a foreign county (with the exception of Canada, Western Europe, Australia and New Zealand) during the last three years for a consecutive period of five or more months one of the following is required:

  • Evidence of a negative PPD tuberculin skin test within 90 calendar days prior to registration application and certified by a licensed physician or department of health.
    OR
  • Evidence of a normal (negative) chest x-ray taken within 90 calendar days prior to registration.
    OR
  • Documentation of a negative symptom screen/risk assessment from the health department or private physician for students who:

    1. present written documentation of having completed prophylaxis treatment for tuberculosis;

    2. have been enrolled in any U.S. public school immediately prior to enrollment in Fairfax County and have a documented history of a positive PPD and a negative chest x-ray from the previous school;

    3. have a history of a positive PPD and a negative chest x-ray more than 90 calendar days prior to registration.  If the PPD tuberculin skin test result is inconclusive the student may be conditionally enrolled with a temporary medical waiver for the PPD or chest x-ray, but the results from a second tuberculin test and/or chest x-ray must be provided within 30 days of the first test.

Arrangements must be made to have your child’s educational records sent to Franklin. These records include withdrawal grades, previous grades, special services records, and discipline status.  After the principal has reviewed your child's educational records, course request form, and any additional pertinent information, a class schedule will be generated.

At the time of your initial visit, we would be happy to provide your family with a tour of the building as well as informational materials on a variety of programs and activities offered at Franklin Middle School.

Central Student Registration   This office provides school enrollment for students whose home languages are other than English, foreign exchange students requiring I-20 student visas, all students whose natural parents or legally adoptive parents do not reside in Fairfax County, tuition-paying students and students in foster care placed by other Virginia Counties or by out-of-state agencies.  Call 703 876-5219 or 703 876-5230 for more information and to schedule an appointment.
