Clearly state your name, your student's name and spelling, student's grade, reason for absence and a return phone number.
PRE-ARRANGED ABSENCES
Whenever possible, students are encouraged to prearrange absences at least 48 hours before the absence occurs. All trips including school field trips and college visits are to be prearranged. Medical procedures, except for emergencies, are to be prearranged. Approval for prearranged absences provides students time to complete the missed assignments while they are away and the opportunity to keep pace with the class, while avoiding the requirement to complete make-up and regular work at the same time after the absence. All assignments assigned by a teacher before an approved prearranged absence begins will be due upon the student’s return. Students can obtain a prearranged absence form from the attendance office. Detailed instructions are on the form. Parents and the student’s teachers sign the form and the student must return the form to the attendance office at least 48 hours prior to the date of the absence for approval.
EARLY DISMISSALS
Prior to leaving school early, a student must bring a note signed by a parent or guardian to the front office at the beginning of the school day stating the reason for leaving and the planned time of departure. The student will be given a pink early dismissal/check-out form by the office staff. This will be his/her pass to depart from class at the indicated time. Parents must come in to the office to sign-out their child. A record of each student's check-outs will be kept by the office. In some cases, the office staff may ask that doctor, dentist, court, or attorney appointments be confirmed in writing on office stationery by the professional visited. If a student returns to school again later the same day, he/she must check-in at the office.
