Message to all 5th grade students: You must complete your Hepatitis B Vaccination Series and have had a Tdap booster before entering the 6th grade. Please provide documentation to the school clinic as you receive your vaccinations.
You need the following immunizations upon entering any school in Fairfax County .

• DPT, TD, DT, DTaP--a minimum of 3 doses at least six weeks apart, with one administered after the fourth birthday.
• OPV, IPV-- A minimum of 3 doses at least six weeks apart, with one administered after the fourth birthday.
• MMR--A minimum of 2 doses of measles, one of mumps and one of rubella. The first dose must be administered at age 12 months or older.
• HBV--A minimum of 3 doses is required.
• Varicella Zoster--For children born on or after January 1, 1997, one dose is required after age 12 months.
