| The clinic's goal is to ensure
that your student will enjoy a safe and healthful school year. Please read
the following information carefully. If you have any questions, please call the

School Health Services Include
- Care of sick and injured students and administration of medication according to Fairfax County Public School guidelines
- Coordination of care for students with identified health concerns
- Identification and follow-up of communicable disease
- Vision and hearing screening for K, 3rd, and new to county students
Help the Clinic Provide Care
- Complete and return the Health Information
Form, listing specific
health conditions that may impact your child's school day.
- Complete and return the Emergency Care
Information Form, listing as
emergency contacts someone who can be reached and can pick up your
child should he/she become sick or injured at school. Notify the school
office and the clinic of any changes to the information provided on
this form.
- If your child will have PE restrictions, notify the PE teacher directly.
- Keep a sick child home. Check your child before he/she leaves for school
in the morning. It is difficult to concentrate and perform tasks when
not feeling well.
