Help the Clinic Provide Care

  • Complete and return the Health Information Form, listing specific health conditions that may impact your child's school day.
  • Complete and return the Emergency Care Information Form, listing as emergency contacts someone who can be reached and can pick up your child should he/she become sick or injured at school. Notify the school office and the clinic of any changes to the information provided on this form.
  • If your child will have PE restrictions, notify the PE teacher directly.
  • Keep a sick child home. Check your child before he/she leaves for school in the morning. It is difficult to concentrate and perform tasks when not feeling well.
