We have created two PDF form “bundles” for your convenience.  These bundles will help you reduce the number of times you have to enter the same information on our forms.  We recommend you check each page for accuracy before printing and print ONLY the pages that apply to your student(s).  The two bundles are for: 

1. new students enrolling (or re-enrolling) in FCPS

2. continuing FCPS students (returning to FCPS after the summer break)

The ENROLLING STUDENT bundle is for new students (or those re-enrolling after an absence from FCPS).   This bundle is a large file, if you have a slow Internet connection or an older computer you may have a problem with this combined file.  The forms that make up this bundle are also available individually from the list below.

The form bundles work best when used with Adobe Acrobat software, version 6 or higher.  If you need the FREE Reader software, follow this link for some additional information and the link to the Adobe site for the download.

The CONTINUING STUDENT bundle contains several forms that are commonly used for FCPS students coming back after the summer break.  This file is about 1MB, still a relatively large file that may be more than you want to open.
