IFTA Registration for Non-FCPS Students

Non-FCPS students must provide the following documentation with the completed registration form and payment:

• the completed registration form
• birth certificate
• medical evidence of immunization
• completed and signed "Affirmation" form SS/SE-219
• a most recent 2009-2010 school report card
• a payment of $750, if a Fairfax County resident
• a payment of $950, if not a Fairfax County resident
