Centreville High School will be giving the annual Fairfax Youth Survey to sophomore and senior students November 15th and 16th. The survey will be conducted through English classes and focuses on teen risk factors including alcohol, tobacco, drug use, delinquency, violence, personal health, and eating disorders. If you do not want your child to participate in the survey, you must fill out this opt-out form and send it back to Dave Paganin, Director of Student Services, no later than November 7th.
