Back to School Forms

 

Welcome back to school!

 

Student Schedule Pick-Up/ Form Collection/ Locker Distribution

August 29, from 3pm to 8pm in the Cafeteria.

Lockers will not be issued until all seven signatures on the four forms below have been completed and signed by your parents.  The forms may be returned to your W4 teacher or you may complete the online forms at home and bring them with you on Wednesday, August 29 when you pick up the student schedule.  Families that turn in all of their forms will be provided with their locker assignment and combination.

 

If these forms are not turned in on Wednesday, August 29, they must be returned on the first day of school to the students’ W4 teacher:

 

• Emergency Care Information. If you have to make updates to the Emergency Care Form, please go to Blackboard-Parent View, update the information using the weCare@school link, print it out and bring it with you on August 29th. When you print out the form, you will also get the Health Information Form as part of the printout.
  • If you do not have a Parent View Account on the 24-7 Learning (Blackboard) site, please go to 24-7 Learning and click on Parent Information to learn how to create an account. Once you create an account, it can be used with all of your students through their years in FCPS.

Annandale HS Parking Permit

 

To obtain a parking permit you must read Regulation 2630.6 and complete the forms below.  Submit the completed forms  to the Main Office by 2:30pm, Friday, September 7, 2012 (do not turn in money at this time).
